ShareHope Medicine company would like to provide Comprehensive Integrated Healthcare Services, you could call it CIHS. And build a digital healthcare ecosystem, we did use some telemedicine and telecare method in our services. Though, in my humble opinion, the telemedicine service right now in Taiwan is a temporary plan to cope with COVID19.
More clinics than 7-11 in Taiwan!
As a matter of fact, it is so convinient for Taiwanese to find a clinic or hospitals in Taiwan. There are more than 11,000 convinient stores in Taiwan. However, there are about 17,000 clinics, including western medicine and Chinese medince. In my limited observation, in most cities and metropolitan areas, there was no barrier for general people to get to the doctors.
The value of Telemedicine
Though, in rural areas, for the disabled, the elders who lives at the fifth floor of an old building withoir elevators. Telemedicine and telecare are extremely crucial healthcare resources for them, either it's covid era or pre and post covid era.
How much budget does Taiwan have for Telemedicine?
It's single payer system of medical services in Taiwan. The National Health Insurance decides the reimbursement of medical services. After many discussion, back and forth compromisation, we have 100 million NTD, aroud 3.7 million USD budget a year for the reimbursement in the sector of telemedicine 2021.
Today is 7th March 2022. So we got the data from last year, the first year of telemedicine reimbursement of Taiwan telemedicine. The total amount of reimbursement is 850 thousand NTD, about 31 thousand USD.
How to transform value into renevue?
Part of Taiwanese would take the regular medical services after the Pandemic. From my point of view, the chance and key of developing telemedince and telecare service lies in the services process re-design. If the ''tele'' procedure merges into people's daily life and combine with health checking services, people would use the tele-consulting without limitation. And the "online merges offline" concept would be realized accordingly.
Let's put apart the telemedicine reimbursement of National Health Insurance for a minute. Is it possible for the telemedicine, telecare, tele-consulting, in short the tele-related services company make money without the cash flow from the original system?
Do one more thing and share profits!
In order to make money without braking the existing structure of value chain, we need to make a whole new cycle with stakeholders. For example, I am working on the Smart Medical Mall at Taoyuan Taiwan. There are multiple medical divisions in there, such as Ophthalmology, Nephrology, medical aesthetics, Gynecology and Obstetrics, Smart healthcare services demo site, YesChain Pharmacy, and so forth.
If a person come to the pharmacy to buy a bottle of eye drop. Beside selling the bottle of eye drop, pharmacist could recommend the client to health check center for a free eye checking. Health check center found out that the person get the dry eye syndrome, thus the patient is referred to Ophthalmology, the eye doctor provides nice services for the patient and get paid by the national health insurance.
Ophthalmology could give health check center part of the amount of money paid by the national health insurance. The health check center could share part of the profit with the pharmacy and the pharmacist. It's the cycle and the value chain of medical and care services. Especially, we must share the profit not only with the employer but also the employees. By so doing, employeees would have lots of energy and incentives to do "one more thing" to make a little but great difference daily.
Free services wins you the clients' favor!
Let's talk back to the services about telemedicine and telecare. If we merge telemedicine services into the app of health check service. People could ask phycisians questions by appointments on app for free. If there's something wrong with his or her body condition, he or she could go to the regular clinic or hospital recommended on the app for further examinations. Or he or she could add more checking programs in the health check program next year.
People get the convinience and the free online services. Phycisians get patients referred by the app and get paid by National Health Insurance. Health checking center get more revenue from the more checking programs that clients pay for. And the more revenue the health check center earns, the more free online telemedicine or tele-consulting services the health check center could offer to the clients.
It's just some of the ideas and the projects I am doing right now with multiple healthcare services stakeholders in Taiwan. ShareHope medicine aims to create a sustainable ecosystem of healthcare industry in Taiwan with the integration of vertical domain and horizontal collaboration with multiple stakeholders.
